Forms

This page contains all of the forms you will need to open and manage your Raymond James Charitable Endowment Fund accounts. All of the forms are in PDF format, and you will need Adobe Acrobat to open and view them. If you do not have this software, you can download it from the Adobe web site.

Submission Instructions

All activities required to open and maintain your RJCEF account(s), with the exception of donor advised fund grant recommendation (see more about this below), can be accomplished by printing and either faxing or mailing the appropriate form to the RJCEF.

Donor Advised Fund Account Forms

Account Set-Up Forms

To open an account, please complete a Donor Account Application. The minimum initial contribution is $10,000. Please review the Asset Transfer Instructions to see if additional forms are necessary. To establish your intentions for a planned gift to the Raymond James Charitable Endowment Fund, please complete the Planned Gift Donor Account Application.

Account Maintenance Forms

To request a change of donor advisor on your account, complete the Donor Advisor Change Form. To request a change in how your account is invested, complete the Request for Change of Investment Form.

Your personal information (name, mailing address, etc.) and password can be changed by clicking the Account Access link at the top of the page and accessing the North Star 500 (NS500) donor advised fund account interface. Read the North Star 500 User Guide for more information.

You have the ability to receive your DAF account statement via email or have your financial advisor submit grant recommendations for you. To implement these items, complete and submit the North Star 500 Authorization form.

To Submit Grant Recommendations from your RJCEF Donor Advised Fund Account:

  1. Click the Account Access link at the top of this page and enter the NS500 donor advised fund account interface.
  2. Enter your user ID and password to enter the site. If you do not have this information, contact your financial advisor or the RJCEF at 866-OUR-FUND.
  3. Click the My Grants tab on the left side of the NS500 page.
  4. Click the Recommend a Grant button located at the top of the grants page.
  5. A list of your favorite charities will display. You can either add a new charity from a list of already approved charities or suggest a new charity that will require approval.
  6. Click the New Grant button located to the right of the charity of your choice. Complete the Making a Grant form and submit it.

Pooled Income Fund Account Forms

Account Set-Up Forms

To open an account, please complete a PIF Account Application. The minimum initial contribution is $20,000. Please review the Asset Transfer Instructions to see if additional forms are necessary.

Account Maintenance Forms

To request a change of donor advisor on your account, complete the Donor Advisor Change Form.


Charitable Gift Annuity Account Forms

Account Set-Up Forms

To open an account, please complete the Charitable Gift Annuity Application. The minimum initial contribution is $50,000.


Charity Advised Account Forms

Account Set-Up Forms

To open an account, please complete the Charity Advised Account Application and Adoption Agreement. A board resolution must also be completed and signed by the organization’s Secretary. A sample board resolution is part of the account application form. A minimum initial contribution of $25,000 is required.


Asset Transfer Instructions

These instructions cover contributions of cash and public securities to the Raymond James Charitable Endowment Fund (RJCEF). If you do not find instructions for your contribution type, or if you need assistance, please call your financial advisor. If you currently have an account with the RJCEF and wish to make an additional contribution, please complete and submit an Additional Contribution Form.

Stock Certificates Application, certificate(s) and Stock/Bond Power(s) (we suggest a mail method requiring a receipt and mailing the certificate(s) and Stock/Bond Power(s) in separate envelopes). Submit original Letter of Instruction (see “Gift of Stock or Bond Certificate”). Endorse the Stock/Bond Power(s) exactly as name(s) appear on each certificate and we will complete the rest of the form. Sign one Stock/Bond Power per certificate. Photocopy the Stock/Bond Power as needed.
